I certify that I am over the age of 18. I understand that in order to participate in the ritual ceremonies I must become a church member of the CAYA Sanctuary and complete the medical intake process prior to attending. I understand that my health and well-being are of the utmost importance and that my health may prevent me from imbibing the sacrament. I understand that I am welcome to attend even if I am unable to imbibe the sacrament of the church.
